HIPAA Notice of Privacy Practices
How your medical information may be used and shared, and the rights you have over it.

Our duty to protect your information
WeCare Medical Specialty Group is required by law to protect the privacy of your protected health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. Protected health information is information about you, including demographic details, that can reasonably be used to identify you and relates to your past, present or future physical or mental health, your treatment, or payment for that care.
How we may use and share your information
We may use and disclose your health information without your written authorization for the following purposes:
- Treatment. To provide, coordinate and manage your care, including sharing information with other clinicians involved in treating you, such as a referring physician, imaging centre, physical therapist or surgeon.
- Payment. To bill and collect payment from you, your insurer, a workers' compensation carrier or a motor vehicle claim, including verifying benefits and obtaining prior authorization.
- Health care operations. To run our practice, including quality review, training, licensing and business management.
- Appointment reminders. To contact you about appointments, results or treatment alternatives, by phone, text or email using the contact details you give us.
- As required by law. Including public health reporting, communicable disease control, suspected abuse or neglect, health oversight, court orders and subpoenas, law enforcement requests, coroners, organ donation, workers' compensation claims and serious threats to health or safety.
Uses that always require your written authorization
We will not use or share your information for marketing purposes, sell your information, or release psychotherapy notes without your written authorization. If you give authorization, you may revoke it in writing at any time, and we will stop, except where we have already acted on it.
Your rights over your health information
- Get a copy. You may inspect and request an electronic or paper copy of your medical and billing records. We will usually respond within 30 days and may charge a reasonable, cost-based fee.
- Ask us to correct it. You may ask us to amend information you believe is incorrect or incomplete. We may deny the request, and will tell you why in writing.
- Request confidential communication. You may ask us to contact you a specific way, or at a specific address. We will say yes to all reasonable requests.
- Ask us to limit what we share. You may request a restriction on certain uses or disclosures. We are not required to agree, except where you pay for a service out of pocket in full and ask us not to share that information with your health plan — there, we must say yes.
- Get a list of disclosures. You may request an accounting of certain disclosures we have made in the six years before your request.
- Get a paper copy of this notice. On request, at any time, even if you agreed to receive it electronically.
- Choose someone to act for you. A person with medical power of attorney or a legal guardian may exercise these rights on your behalf.
- Be notified of a breach. We must tell you if a breach compromises the privacy or security of your information.
Changes to this notice
We may change this notice, and the changes will apply to all information we hold. The revised notice will be posted on this page and available in our offices.
How to raise a concern
If you believe your privacy rights have been violated, contact our office at (973) 996-2990 or info@wecaremedical.us. You will never be retaliated against or penalised for filing a complaint.
You may also file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, at 200 Independence Avenue S.W., Washington, D.C. 20201, by calling 1-877-696-6775, or at hhs.gov/ocr/privacy/hipaa/complaints.
